Maximize Your Health-Free

About Us

Our program is one of 14 Self-Management Programs across the Province of Ontario. We offer Self-Management training and workshops to participants, caregivers and health care professionals across the Mississauga Halton Region.

In Mississauga Halton Region we envision individuals with chronic conditions working as partners in their own healthcare. We provide programs and services that enable people to take control of their own health by gaining the confidence, skills and knowledge necessary to manage the physical, social and emotional aspects of life with chronic conditions.

We also offer evidence based trainings for health care professionals that will help them support their patients. Trainings are focused on developing communication skills to motivate patients to make health behavior changes.

The Self-Management Program is funded through the Mississauga Halton LHIN (Local Health Integration Network) and Hosted by Halton Healthcare Services. Our current program offerings include:

  • Stanford Chronic Disease Self-Management Program workshops (In our region titled “Maximize Your Health” workshops in Chronic Pain & Chronic Conditions )
