Topic: SCOPE Mental Health Program: How It Can Help Your Primary Care Physician Help You
The goal of SCOPE (Seamless Care Optimizing Patient Experience) is to assist in the co-management of patients with mental health and additional needs by providing primary care providers with system navigation to mental health and additional services and brief primary care provider (PCP) to psychiatrist consultation The Nurse Navigator will assess patient’s service needs and provide assistance with service navigation, connecting patients with appropriate resources, and ensuring referrals are in place.
